Thankfully the one time I fully and completely screwed up an instrument approach I was in coastal Florida and I got away with it. I confused the point where I joined the NDB approach course with the point where I was supposed to begin the descent to minimum altitude. It was only a coincidence I had just passed the only obstacle taller than the 400 feet above ground level I could have run into. You don't get to do that around the mountains more than once in a life. It still makes my knees start shaking when I think about what could have happened.
